Please fill out this form for Jersey Selection.

Number selection goes from oldest to youngest, based on date of birth. To prevent girls on the same team from having duplicate jersey numbers, a number is issued only every four years (ie. #15 graduation year 2015, the next available year of #15 is 2019). Thank you for understanding.

Samples of each item are on display at Quali t Screening to check for proper sizing.

The player uniform will remain the same every year unless it is discontinued.
