Please see the CampMinder section of this website, and make sure you are able to log into CampMinder. All forms can be accessed through that system, and all paperwork needs to end up saved there.
We've brought over any relevant paperwork from 2024. If you don't see it in your account, then we don't have it and you should submit it. If a form is marked incomplete, it is out of date or missing something. If you aren't sure what that is, email Joanna at joannacampkinderland@gmail.com. A few notes and instructions:
W4 :Please fill out and upload to CampMinder. You are welcome to fill it out digitally, but it may not allow you to sign it. If so, print it out, sign it, then scan and upload.
FULL TIME STUDENT: Just a quick form asking if you're a full time student, which has tax implications.
CONCUSSION AWARENESS TRAINING: Massachusetts requires that all staff who work directly with campers complete this online concussion safety course every year. It takes about 20-30 minutes. It will require you to first create a profile if it is the first time you are taking the course. If you completed it in the past you can just log in. Please be sure that the course you take is called "HEADS UP to Youth Sports Coaches". Here is the link to begin the course: Concussion Awareness training. If you have any questions, read the FAQ sheet link in CampMinder.
When you complete the course, you will be able to download a certificate. Please then upload that certificate to CampMinder.
I9 Form: If the I9 is not listed in green in CampMinder, please fill one out now (they are only good for 3 years). You can download the I9 form from CampMinder. Please follow these instructions carefully:
You can either download it and fill it out by hand, or fill it out digitally.
Once you have the form, fill out Section 1 completely. Don't forget to sign and date this section (in the middle of the page).
Fill out Section 2 - you will need your identification documents. You should use a valid passport if you have one (List A). You can then leave the other lists blank. On List A, the issuing authority is the US Department of State.
If you don't have a passport, you will use BOTH a photo ID and social security card (List B and C). You can use a driver's license, non-driver state ID card, or a school ID card if it has a photo. If you are 17 and don't have one of these, you can use a school report card instead. On List B, the issuing authority is your state's DMV, or your school. On List C, the issuing authority is the US Social Security Administration.
At the bottom of the page you will see a section that begins with the word Certification. Leave this section blank - camp fills out this part.
When you are done, save the file as a PDF. Please don't use a jpg. Upload the file to CampMinder. If you have questions about how to fill out your I9, you can email Joanna at joannacampkinderland@gmail.com.
IDENTIFICATION: Please upload a copy of the IDs that you used to Camp Minder (these can be jpgs).
MEDICAL EXAM FORM: Massachusetts now requires that all staff have a medical exam from within 18 months of camp (January 2024 or more recent), This must be filled out by a medical provider. Most medical offices have their own form for school/camp that they can print out for you, or that can be accessed in your portal. If not, you can download a blank form from CampMinder (but please check with them first - it saves everyone a lot of time).
IMMUNIZATION RECORD: This will be listed in green if we have a complete one on file for you. If it is marked incomplete, it means you are missing something. Email Joanna at joannacamokinderland@gmail.com if you are not sure. Here are the MA requirements for staff (please note that tetanus vaccines must be given every 10 years):
MEDICAL RELEASE FORM: If you are UNDER 18 years old, you MUST have your parents sign this form; and then please upload it to CampMinder.
NO ONE MAY WORK AT CAMP WITHOUT CURRENT MEDICAL FORMS.
