The initial member organizations were identified by a social network analysis related to hospital readmissions or represented a relevant scope of practice in the community. By consensus, the Coalition has remained small guided by the following values:

• Trust building is critical to success

• Data sharing is protected

• Accountability is enhanced

New members are suggested by the coalition itself when a specific scope of practice is needed to enhance our work. Other organizations who are interested in participating in the coalition, may contact the facilitator, Helen Sampson (Helen.sampson@kenoshacounty.org). Please include information on your organization's experience with care transitions and any specific expertise, data or intervention that your organization is prepared to contribute and the coalition members approve by consensus. Each organization chooses its own representative(s) to attend the coalition with the understanding that those individuals should be able to impact their organization's systems (i.e. administration or clinical) and provided consistent attendance.

The Coalition has several workgroups which are actively working to further understand shared concerns and identify and implement strategies for improved outcomes.
