mPROVE (Mobile Patient Reported Outcomes for Value and Effectiveness) is a research app that allows a person to complete health-related surveys on their smartphone and share the results with their healthcare provider.
Data collected as part of the study will be stored and used as described in the informed consent signed before joining the study.
Types of data collected include: contact info, account ID, survey responses, data generated during a customer support request, app usage data (e.g., app launches), diagnostics.
Identifiable data collected to determine eligibility will be stored on a secure server behind UCSF firewalls. PRO data will be stored in an encrypted state on a secure server behind UCSF firewalls that is fully HIPAA compliant.
Researchers will use your information to conduct this study. Once the study is done using your information, we may share them with other researchers so they can use them for other studies in the future. We will not share your name or any other personal information that would let the researchers know who you are.
We will not ask you for additional permission to share this deidentified information. We will do our best to make sure that the personal information gathered for this study is kept private. However, we cannot guarantee total privacy. Your personal information may be given out if required by law. If information from this study is published or presented at scientific meetings, your name and other personal information will not be used.
Authorized representatives from the following organizations may review your research data for the purpose of monitoring or managing the conduct of this study: Representatives of the Agency for Healthcare Research and Quality, Representatives of the University of California
While we will make every effort to ensure that data from the app remains secure, data may be lost due to loss of your smartphone, hardware or software failure, or if the server is breached.
You have the right to decline to be in this study, or to withdraw from it at any point without penalty or loss of benefits to which you are otherwise entitled.
The app includes an email and phone number for reaching study staff for any research study or technical issues.
