Forms

Here are all the forms you will need to print and complete before our first appointment. If you are using your EAP, or paying out of pocket, you do not need to complete the insurance billing form.  Because I am not currently seeing clients in the office, you will have to return them to me either electronically or by mail. You can send them by fax as well. We can discuss options for how to get the forms to me. Let me know if you have any trouble with this, and we will work it out together.
