Bus Release Form

Each swim meet athletes will be bused to and from the meet as a team. If a swimmer needs to leave a swim meet with a parent for another event after the meet is over, the swimmer will need to fill out the Bus Release Form and return it to Coach Shupe before getting on the bus to go to the meet. Make sure to fill this out early since it requires a signature from an administrator.
