1: Fill in the D39C Club Interest Form. Last day to register is Friday, September 21st, 2018 by midnight.

Student Enrichment Club Interest Form

2: Follow instructions within Student Enrichment Club Interest Form link and sign Permission/Waiver form.

If you signed Student Enrichment Club Interest Form prior to Sept 10, 2018, you will receive email instructions from D39C Collaborative about

Permission/Waiver form.

3: Suggested Donation of $25. This will cover competitions registration, team t-shirt, year end party & awards etc. Make check payable to "The D39C Collaborative" (Memo line: Math Club & Student’s Name). Please bring donation check to the "Math Club"/SE drop box located in the Welcome Center by end of day, Fri Sep 21st.

We need many coaches and volunteers in order to provide a quality program to our students.

Weekly meeting will start in TBD October date depending on number of coaches and volunteer clearance.
