Corvallis 406 Throws Camp Forms

Camp is on Thursday, June 20, 2024

If you are having problems with google forms, download the brochure* & parental release form.  

If mailing, please postmark by June 1 and contact Garrett to reserve your spot.

Google Form is the preferred way to register for camp. Login to google before starting form.

Google Form will be closed on June 10 at midnight.  Depending on number of participants, google form may be closed sooner.
