3. Please provide right to work documents:

a. Identification (School or State ID)

b. Social Security Card (Please ensure that it is signed)

c. Birth Certificate

d. Work Permit (Required for Minors only; Please complete attached form and provide the completed document to your school.)

e. Proof of Residency, including a post marked date. (Ex. Report Card, Transcripts, or any mail that is under the applicant’s name)
