Below you will find the following helpful links and forms:

• Professional Development Activities Chart (outlines all activities, necessary verification, and limitations for professional development credit)

• Links to Forms (links to all of the forms you need to construct your Individual Professional Development Plan and to request and verify non-traditional credit toward your licensure renewal.   Clicking the links will prompt you to make a copy of the specific Google Doc which you must save and edit in your individual Google Drive.)  

• Educator Licensure Renewal Verification Form (to be initialed annually at the end of each school year to document verification of credits earned) 

• Educator Leaving an Ohio Local Professional Development Committee (LPDC) Verification Form (necessary anytime an educator leaves an LPDC - This form becomes a part of an educator's IPDP and must accompany their plan when entering a new LPDC.)

• Significant Tie cover sheet (for educators who are not currently under an LPDC yet have a significant tie to a COCSC member school and wish to be served by its LPDC - Examples of a “significant tie” are: substitute teacher; tutor; consultant; educational aide. This form must be submitted to the LPDC for approval.)
