The goal of this project is to understand how to create an accessible form. You will create a version of a form that is accessible. You'll report on your findings by submitting a URL to your form and writing up a one-page report. WebAIM has documentation, with examples, on how to create an accessible form.
We have provided three examples of the same form you can base your work off of.
First, try to use the inaccessible form and results with a screen reader. You might try VoiceOver on the Mac, WebAnywhere, or ChromeVox. Also try using it without a mouse (keyboard only). Put some notes about particularly problematic things you discovered in your write up.
Next you should create a new version of the form and results page that is more accessible. Your results page should just be a sample of what should be produced if the form is filled out as in our image of the incorrect filled out form. You can edit the HTML and fix it, or write your own HTML. Make a list of the things you fixed and how, and put them in your write up.
When you are done, try to use the form with a screenreader, and no mouse. Put some notes about what has changed in your write up.
