Physical Examination and District Health Forms

All of the following items in the Sports Physical Examination packet must be completed and returned to the Activities Director (through the High School Office or through the coach/sponsor) and approved by the school nurse before any student may practice or compete in a school-sponsored activity:

Sports Physical Examination (Complete Packet)

• Physical Examination (use DHHS form provided)

• Health History (use DHHS form provided)

• NSAA Parental Consent Form

• Athletic Insurance Coverage Form

• Physical Fitness, Health and Injury Procedure Form

• ImPACT Test (Baseline Testing for Diagnosing Concussions)
