Gift Confirmation Form

There are two main documents to confirm your gift to Ben Hill UMC.

1. Legacy Endowment Gift Confirmation Form 

   • This form is specific for gifts directed toward Legacy Endowment Giving.

2. Gift Donation Form

   • This is a general form to designate a type of donation, the designated ministry use and the amount.
