Ohio Safety Council Enrollment Form

Please complete a Ohio Safety Council Enrollment Form, then submit to Jessica Dodson-Pierce by either emailing it to arsc@perrycountyohio.net or fax it to (740) 342-1685.

The Ohio BWC Safety Council Rebate Program requires annual enrollment in a local safety council by July 31, 2023.

New members are encouraged to enroll at any time of the year to benefit from the guest speakers. Just keep in mind you may not be eligible for the rebate until the following year, when you are able to attend the required amount of meetings.
