Teacher Request Form
Please download and complete the form below to request the Boosters support and/or funding of Avon Middle High School club, athletic, grade or individual classroom activity, enrichment program, classroom or school supplies which are not covered under the school budget. Please be sure to provide a detailed description of your request as well as the total cost. All requests should be passed into and approved by the principal of the Avon Middle High School, who will then present the request on your behalf at a scheduled Boosters meeting. The Boosters meet on the second Wednesday of every month. The requests will be reviewed and approved based on available funds and majority vote.
