• Registered Nurse (RN) license from the state board of nursing
• Baccalaureate degree from an accredited college or university
• School Nurse Services Credential from state commission on teacher credentialing
• School Audiometrist certificate from state department of health services
• Experience in the areas of pediatrics, public health, health promotion, assessment and referral
Examples of Responsibilities & Activities
• Assess and treat sick and injured students using clinical assessment skills and judgment based on community standards and ongoing continuing education
• Provide annual in-services to staff
• Review annual Health Information sheets completed by parents and document pertinent data in computer database; sharing pertinent information with school staff to ensure student safety at school
• Stock health office and classroom first aid kits, maintain health office budget, order supplies as necessary
• Review immunization record of all incoming students; refer for additional immunizations as needed
• Complete and submit required annual immunization reports to the State
• Set up medication book; contact parents and physicians of students who require medication at school; train office staff in medications (annually and as situations change); monitor that students who have medication ordered are receiving them as specified; monitor students for side effects and efficacy of prescribed medication; contact parent/physician as necessary
• Organize and complete state mandated screenings (Vision - kindergarten, 2nd grade, 5th grade, 8th grade; Color Vision - 1st grade boys; Hearing - kindergarten, 2nd grade, 5th grade, 8th grade; and screen students who were not screened at previous school when they were due and those new to the district). Rescreen students with deviations from accepted norms. Complete referrals as indicated. Assist parents with obtaining vision, dental or medical care. Contact parents until problems resolved, treated, or cleared by physician. Re-screen students once they have received their glasses
• Complete mandatory nursing assessment reports on students for initial and triennial IEPs, and for annual and supplemental IEPs if there are any health concerns
• Review Health Information cards and computer database to case-find all students with diagnosed asthma or history of asthma; determine need for inhaler at school, obtain release of information and physician orders; monitor use of inhaler; instruct student/parent regarding asthma control, prevention, and medication use
• Assist with registration – immunizations, medications at school, pertinent medical histories, obtain necessary information, documentation and physician orders for students with life threatening allergies, diabetes, seizure disorders, asthma, etc.
• Contact parent and physician for all students who need special diets at school; provide school dietician with written prescription to ensure student’s safety; follow up with parent for date diet will go into effect
• Communicable disease control – notify SDUSD Nursing Department Communicable Disease Nurse as required by state law. Work with school site to notify potentially affected staff and students; counsel and facilitate follow up as indicated
• Provide information on nutrition and activity to students and parents as requested or indicated
Provide health education and counseling to students, parents, staff (including, but not limited to, pregnancy, drugs/alcohol, smoking cessation, eating disorders, self-injury)
• Instruct students who present to health office on health related activities (wound care, signs of infection, injury healing, role of sleep and nutritious foods, asthma prevention, use of inhalers and peak flows, asthma pathophysiology, etc.)
• Review PE excuses from physicians and parents; complete adjusted physical education forms and distribute to PE teacher and counselor
• Research health and wellness related issues for staff and students; provide information as requested or indicated
• Provide resource materials for pertinent health issues (i.e., pertussis outbreak, flu and cold information during peak flu season)
• Provide health assessments (blood pressure, ear exam, vision screening, auscultate lungs, etc.)
• Attend required meetings and continuing education courses to ensure continued professional competence
