All students with an IEP attending, or planning to attend, a private school within the SHUFSD for the 26/27 school year must complete the following forms no later than June 1, 2026. IEPs are held by the SHUFSD for private schools within our district limits.
Any classified student who has applied, intends to apply, or has been waitlisted to St. Anthony’s needs to complete the forms below by June 1st. If you are accepted after June 1st but have not submitted the paperwork below prior to June 1st, you will not have an active IEP/IESP for the 26/27 school year and no services/accommodations will be considered for the student. There are no exceptions to this rule.

Request for Special Education Services through an IEP
Release of Information please include the following under "to be released between:"
Home School District
District Holding IEP (if different from Home School District)
Metro Therapy (service provider and evaluator for SHUFSD)
Name of Parentally Placed School, i.e. St. Anthony's High School
The release must contain all of the above listings or it will be considered incomplete. There is no need to include the addresses and phone numbers for the above listings. If you include anyone else - a person, practice, etc., please include an address and phone number.
Parentally Placed CSE Registration Form 2026/2027
Must be completed in order to register your child as a student with an IEP being held by the South Huntington Union Free School District.
No registration for a student with an IEP will be considered complete until all of the above forms have been completed and received in our office by the June 1st deadline. There will be no exceptions to this rule.
You will receive a confirmation email from pmcardle@shufsd.org confirming receipt once all three forms have been received.
If you do not receive a confirmation email from Patti McArdle by June 1st, your child will not be considered a classified student with an IEP or IESP for the 25/26 school year. No exceptions will be made to this rule.

Students with 504s
504s are held by your home school district and managed by St. Anthony's directly. Please request your home district share the 504 on Frontline with St. Anthony's High School no later than August 1st. Also email a copy of your child's 504 to the Director of Guidance and Student Services, Chuck Menzer at cmenzer@stanthonyshs.org.
Transfer In Meetings for Students with IEPs
Transfer in meetings are held via Google Meet (telephone or video). A Google Meet Invite will be emailed to you with the information to join the meeting. The meetings are approximately a half hour and we hope to begin scheduling them in late May/early June.
Please add the following email address to your known contact list, pmcardle@shufsd.org, to be certain you don't miss anything that may sent over the next few years. Also please be sure to open any mail you receive from South Huntington Union Free School District as this may be related to your child's IEP.
If any personal information (i.e. telephone numbers, home address) changes over the time your child is attending St. Anthony's, please be sure to contact Patti McArdle at pmcardle@shufsd.org and let her know so we can change our system records as well. Meeting invitations (Google Meet) are emailed to the parent. ADOBE SIGN is used for documents needing consent from parents, so you will receive emails from ADOBE SIGN as well. If at any time your child will no longer be attending St. Anthony's, please contact Patti McArdle so she can send all relevant Special Education information over to the district your child will be attending.
