  • NYS Health Examination Form

  • Immunization Certificate

• Proof of Residency

  • Proof of Ownership: Deed/Tax Records or Proof of Rental: Current Rental Agreement

  • If you do not own a home or have a lease to rent an apartment, room, private home, you will need the following documents: 

Affidavit of Tenant (from parent) & Affidavit of Owner/Landlord- notarized - Click: English / Spanish
