IFPP™ Privacy Policy

Integrated Functional Pain Program™ ("IFPP," "we," "our," or "us") is operated by Shokat Wellness Academy LLC. This Privacy Policy describes how we collect, use, store, and protect your personal and health-related information when you use the IFPP mobile application ("App"). Your privacy and the security of your data are of the utmost importance to us.
1. Information We Collect
When you use the IFPP App, we collect the following types of information:
Account Information: Name, email address, and authentication credentials provided during enrollment by your healthcare provider's clinic.
Health and Wellness Data: Daily self-reported information including pain levels, pain location, sleep quality, mood, activity level, and breathing exercise completion. This data is collected to support your pain self-management and to enable Remote Therapeutic Monitoring services provided by your healthcare provider's clinic.
Educational Progress: Lesson completion status, program progression, and curriculum engagement within the 8-week educational program.
Procedure Follow-Up Data: If assigned by your healthcare provider, post-procedure self-reported outcomes including pain levels, functional improvement, and recovery progress.
Device Information: Device type, operating system version, and push notification tokens used to deliver reminders and program notifications.
2. How We Use Your Information
We use your information for the following purposes:
To deliver the IFPP educational curriculum and self-management tools to you. To enable your healthcare provider's clinic to monitor your progress and provide Remote Therapeutic Monitoring services. To send you daily reminders and program notifications. To generate aggregate, de-identified outcome data that helps healthcare providers evaluate treatment effectiveness. To improve the App's functionality and user experience.
3. How We Share Your Information
Your personal health information is shared only with the healthcare provider clinic that enrolled you in the IFPP program. This is necessary for your clinic to deliver clinical care and to document Remote Therapeutic Monitoring services.
We do not sell, rent, or trade your personal information to any third party.
We do not share your individually identifiable health information with advertisers, data brokers, or any entity unrelated to your clinical care. We may use aggregate, de-identified data (which cannot be traced back to you) for research, product improvement, or reporting purposes.
4. Data Storage and Security
Your data is stored on HIPAA-compliant infrastructure provided by Amazon Web Services (AWS). We implement the following security measures to protect your information:
All data is encrypted at rest using AES-256 encryption. All data is encrypted in transit using TLS (Transport Layer Security). Our database is not publicly accessible and is isolated within a private network. Automated daily backups are maintained with 7-day retention. Access to your data requires authenticated credentials. Audit logging tracks all system access.
We have executed a Business Associate Agreement (BAA) with our cloud infrastructure provider as required under HIPAA regulations.
5. Data Retention
We retain your personal and health data for as long as your account is active and as required to fulfill the purposes described in this policy. If you or your healthcare provider request account deletion, we will remove your personally identifiable information within 30 days, except where retention is required by law or for legitimate clinical documentation purposes.

You have the right to:
Request access to the personal data we hold about you. Request correction of inaccurate personal data. Request deletion of your account and associated data. Opt out of push notifications at any time through your device settings. Contact us with questions or concerns about your data.

7. Children's Privacy
The IFPP App is not intended for use by individuals under the age of 18. We do not knowingly collect personal information from children. If we become aware that we have inadvertently collected data from a minor, we will promptly delete that information.
8. Changes to This Privacy Policy
We may update this Privacy Policy from time to time to reflect changes in our practices or applicable regulations. We will notify you of any material changes through the App or by other appropriate means. The "Effective Date" at the top of this policy indicates when it was last updated.
