The first step in getting assistance for a student is to complete the Teacher/Staff Referral Form. After the initial referral is received, personalized observation checklists are sent to the student's current teachers. It is important to note that parents and guardians will have the ability access to these checklists.
Students can also refer other students using the Student SAP Referral Form. These referrals are confidential and anonymous.
Changes in Student Behavior
While there are many different ways in which a student can show a need for SAP, some commonly identified changes are:
Withdrawn or lonely
Changes in energy levels
Cries frequently/appears sad or upset
Expresses hopelessness/helplessness
Distractable/short attention span
Negative self-talk/criticizes self
Themes of death in writing and art
Appears Angry
Frequent discipline referrals
Pattern of truancy or absences
No longer participating in activities once enjoyed
Changes in friends
Deteriorating appearance
Openly talks about alcohol or drug use
Trouble getting along with classmates or adults
Physically and/or verbally abusive toward others
Drop in grades along with multiple changes in behavior
