Tryout Forms- ALL FORMS ARE ON DRAGON FLY

The following forms MUST be filled out/uploaded to DragonFly before you will be allowed to tryout:

-Birth Certificate

-2018 Physical Form - older versions will no longer be accepted.

-Concussion Form - filled out on DragonFly

-AHSAA Release Form - filled out on DragonFly

-Sportsmanship Certificate
