AHSAA Eligibility

All AHSAA Eligibility will be completed via DragonFly Max! All forms will be completed electronically except for the Revised 2018 Form 5 (Physical Form). Parents may complete the front of the form electronically via DragonFly Max or may upload the completed, signed form. The back of the form, once completed fully by an MD or DD, will be uploaded to DragonFly Max. Below are instructions for DragonFly Max (SCHOOL CODE included) and a copy of the Physical Form.
