SFDPH Community Health Programs for Youth (CHPY) is open for FREE school physicals and sports physicals for patients ages 12-25. Please call for an appointment, and complete the attached registration paperwork prior to the appointment. Please note, if student has received a physical in the last year they do not need another physical, they merely need to reach out to their provider to fill out the forms.
