This applies to both medication that is stored at school and that which is carried and self-administered by students, such as inhalers, emergency allergy medications, and/or pain medication.

Please consult with Nurse Casey (caseym1@sfusd.edu) as to which forms are required for submission.
Completed forms may be emailed to Nurse Casey or returned to school with your child.
Note: Any form requiring a health care provider's signature should be signed by a health care provider licensed in California, not any other state or country (CA Ed Code 49423)
SFUSD requiere que todos los estudiantes que toman medicamentos en la escuela tengan un formulario completo firmado por sus padres / cuidadores y su médico.
