To participate in any Sport at Marina MS, you will need to have the Pre Participation Physical completed before tryouts.
Need another language? Visit the CIF webpage (Medical Forms) where you can find other languages including Chinese and Spanish.
Pre Participation Physical - Each school year athletes need to have a completed sport physical. This can be done through your health care prover or an outside organization. Dignity health performs sports physicals and is in very close proximity to the school. Additional sites for sports physicals appointments can be made with CARBON HEALTH. Every student will need to have the Pre-participation Physical before participating in any athletic events.
Marina Athletics will be moving our athletic paperwork to a digital format. We are now asking parents/guardians to use the website: Homecampus.com to complete all athletic paperwork for each school year. The forms will remain the same but will now be a digital signature. Moving to Home Campus will allow everyone to stay updated with athletes being cleared for each sport. Student Athlete SFUSD SPORTS Physical Form (requires doctor signature. You will upload page 4 of this form to the athlete's Home Campus account.)
Athletes can participate in tryouts once they are cleared on Home Campus.

SFDPH Community Health Programs for Youth (CHPY) is open for school physicals and sports physicals for patients ages 12-25.
Please refer your students to one of our clinics below, encourage them to call for an appointment, and ask them to complete the attached registration paperwork prior to their appointment.
These services are offered with no charge to the student or family.
Please note, if student has received a physical in the last year they do not need another physical, they merely need to reach out to their provider to fill out the forms.
