ADOPTION QUESTIONNAIRE

So, you are looking to adopt a dog? How PAWsome! To be considered for adoption, you must:

• Be 21 years of age, or older.

• Live in the Galesburg, Illinois area.

• Have the knowledge and consent of your landlord (if renting).

• Agree to a home visit, sign an adoption contract, and pay our adoption donation.

• Note that completion of this application does not guarantee adoption of a Serendipittys Sanctuary of Hope animal.
