CONSENT FOR USE OF PHOTOGRAPHS AND FILM (Persons aged under 18 years)
During your engagement with Kids SecuriDay, we may may take photographs and/or film in which the Minor’s image is included.
Kids SecuriDay intends to share the photographs and/or film taken which include an image of you with its affiliates and sponsors (‘Affiliates and Sponsors’).
Purposes for collecting personal information
Kids SecuriDay intends to produce promotional material for the purpose of:
Publicising activities and programs of Kids SecuriDay to target audiences, including potential sponsors; and
Promoting the outcomes of sponsored activities and programs to Kids SecuriDay and Affiliate and Sponsors’ target audiences.
These images may include the Minor’s personal information collected in photographs and/or film in the distribution of promotional material through media including:
Electronic and printed advertisements;
Televised activities including news broadcasts, advertisements and promotional features;
Internet sites (World Wide Web), including social media sites such as Facebook, Twitter and YouTube
Request for consent
Kids SecuriDay seeks your permission to:
Take photographs and/or film in which the Minor’s image is included; and
Use the photographs and/or film taken of you, for any of the purposes cited above.
Kids SecuriDay may also provide the photographs and/or film to its affiliates and sponsors as described above.
By providing your consent to the disclosure of the photographs and/or film to our Affiliates and Sponsors, you acknowledge that if an Affiliate/Sponsor handles the photographs/film in breach of the Australian Privacy Principles, Kids SecuriDay will not be liable under the Privacy Act 1988. You further acknowledge and understand that Kids SecuriDay Affiliates/Sponsors are not subject to the Privacy Act.
You do not have to give your consent if you do not want the Minor to be photographed or filmed, however if you do not provide consent the Minor may be asked to wear an identifying sticker to ensure they are not captured in the photographs and/or film. The Minor may also be seated out of view of photographers/film crew etc.
Privacy information
The personal information on this form is requested in order to obtain your consent for the use as outlined above. The information will be used by Kids SecuriDay to verify that proper consent has been provided in accordance with the Privacy Act 1988.

5. I understand that Kids SecuriDay does not guarantee that the photograph and/or film will necessarily be used for any of the purposes or in the media described in this form.
