Athletic PE Contract

College Visit Form

• Juniors and Seniors are limited to 3 days of college visits per school year. 

• The form must be turned in for approval to your assistant principal one week prior to your absence.

FERPA Forms 

• Counselor FERPA form (summer programs, internships, scholarships, college applications). Please note that if you need a letter of recommendation for college applications, you will also need to fill out the College Application Packet located in the College Application Handbook, in your Google Classroom, or on the 12th-grade page.
