Letters of Recommendation 

• If you are a past/current student of Mrs. Smith and would like to request a letter/form of recommendation for colleges/summer programs/etc., please fill out the Google Form & FERPA form below. 

  • The FERPA form is legally required before I can recommend you to any entity outside of SDUHSD. 

    • The top section is for schools on the Common App. If you are applying to more than 10 schools on Common App, please attach an additional page. 

    • The bottom section is for non-Common App schools (eg. the UCs), which lets me know to look out for other emails from those schools.

  • I recommend that you waive your right to review a copy of the letter of recommendation (counselors advise this as well).

• Mrs. Smith will email you at your SDUHSD email address within 1 week with a yes/no and further instructions. (You will get another form to fill out from me before I begin writing your letter.)

IMPORTANT: You must make the request AT LEAST 3 WEEKS before the application deadline. This is necessary to give Mrs. Smith time to write you a good letter! :)
