If you are a past/current student of Mrs. Smith and would like to request a letter/form of recommendation for colleges/summer programs/etc., please fill out the Google Form & FERPA form below.
The FERPA form is legally required before I can recommend you to any entity outside of SDUHSD.
The top section is for schools on the Common App. If you are applying to more than 10 schools on Common App, please attach an additional page.
The bottom section is for non-Common App schools (eg. the UCs), which lets me know to look out for other emails from those schools.
I recommend that you waive your right to review a copy of the letter of recommendation (counselors advise this as well).
Mrs. Smith will email you at your SDUHSD email address within 1 week with a yes/no and further instructions. (You will get another form to fill out from me before I begin writing your letter.)
IMPORTANT: You must make the request AT LEAST 3 WEEKS before the application deadline. This is necessary to give Mrs. Smith time to write you a good letter! :)
