Application

To apply to be a substitute teacher you must hold either an Illinois Professional Educator's License, an Illinois Substitute License, or an Illinois Short Term Substitute License.  To obtain a license, applications can be found at Illinois State Board of Education. 

The following items will be found in your Substitute Teacher application

• Checklist of items needed and timeline of the process 

• Substitute Information Sheet - Grades and subjects you would like to teach

• Applicant History - References (Substitute Application Only)

• Please include an updated resume

• Reasonable Assurance Letter - States that you are not eligible for unemployment benefits

• Student Confidentiality  - Agreement stating you are to not disclose any student information that you come across

• Employment Not Covered by Social Security - States that this job is not covered under the Social Security System

• Eligibility Verification Federal I-9 - Will need two forms if identification, one being a picture ID

• D.C.F.S Mandated Reporter - States that if you suspect child abuse, you must report it to an administrator

• D.C.F.S Authorization for Background Check - States that D.C.F.S can check to see if you have any priors in their system 

• Health Statement/Negative TB test - You will need a Dr.’s signature /statement that you are able to work in the school setting.  You can use a previous statement from up to 1 year.  You will also need proof of a negative TB test no more than 90 days old.  

• Faith’s Law - Passed in 2023  focuses on the prevention of “sexual misconduct”

• Criminal Background Check - Make an appointment online at the North Cook ISC.  Currently the cost to you is $50. (Substitute Application Only)

• Criminal Background Check - Please connect with Christ Barnabas D.74 to make your online appointment with N.T.D.S.E for your fingerprint scan.  The Substitute Cooperative will pay for this. (Substitute Nurse Application Only)

• Direct Deposit Authorization form

• W4 - Federal and Illinois forms

• Copy of your Illinois State Board of Education License: PEL, Substitute or Short Term Substitute - (Substitute Application Only)

• Copy of your Nursing or LPN License and current C.P.R certification for the current school year. (Substitute  Nurse Application Only)
