REQUIRED REGISTRATION FOR ANY ATHLETICS OR ACTIVITIES
ARBITER REGISTRATION: Prior to the beginning of the first day of practice, parents MUST complete Parent Consent Form, Sudden Cardiac Arrest Release Form, Medical Release Form and Concussion Form online AND upload MHSA Physical Form OR Interim Medical Form with insurance card with Arbiter Registration.

☑️ 1. PHYSICAL completed after May 1st to meet the requirements for fall activities OR Interim Medical Form
☑️ 2. PARENT CONSENT AND MEDICAL RELEASE
☑️ 3. CONCUSSION FORM
☑️ 4. INSURANCE CARD OR Proof of accident insurance. Click here for information on purchasing Student Accident Insurance.REGISTRATION APPROVAL: The above forms will be approved by the athletic training staff after all of the above has been submitted as they are received.
