To practice or play in tournaments, the following requirements must be met:
(1) Register on Arbiter;
(2) current physical uploaded to Arbiter (this should include both the "medical history" as well as the actual physical exam form signed by the qualified health care provider);
(3) current Parent Consent & Medical Release form completed in Arbiter;
(4) current Concussion Awareness form completed in Arbiter
(5) copy of health insurance card covering the athlete; and
(6) payment of participation fees (paid through PowerSchool portal). Please DO NOT pay for a "spirit pack" for Golf ... we use a "team store" link with Universal Athletic through which you can order all team gear.)
