• Please note, we are looking for new members to join our committees so we can get your feedback/ideas! Ideally we would like to get at least 2 people to lead each committee. If interested in more information please email madisonpta44@gmail.com.

Volunteer Application

All volunteers at Madison must complete a Volunteer Application before you will be allowed entry to school on a regular school day to volunteer.  You can print the form at the link above and bring to the office with you or fill it out once you arrive. Please review the Volunteer Handbook before the date you plan you volunteering.
