Asthma

In order for you to have the best possible school year, it is important that I have the necessary supplies and information to provide appropriate care.

Your supplies should include:

• Rescue inhaler with prescription label (please ask pharmacy to place label directly on inhaler if it is not being kept in prescription labeled box)

• Spacing device, if available.

• Nebulizer medicine and supplies, if necessary. https://www.cdc.gov/coronavirus/2019-ncov/hcp/infection-control-recommendations.html

• Peak flow meter, if your child’s Action Plan includes peak flow values that will help assess your child’s breathing.

• Completed Asthma Action Plan form.
