PSAL Forms 

Julia Richman Education Complex ( JREC) has established 9 varsity teams that participate in the Public School Athletic League (PSAL), and our teams continue to achieve high levels of success. 

 Girls Varsity Badminton, Boys Varsity Badminton, Girls Varsity Basketball, Boys Varsity Basketball, Boys Varsity  Soccer, Girls Varsity  Soccer, Girls Varsity Swimming  

Boys Varsity Volleyball & Girls Varsity Volleyball

All students participating in a sport must have a PSAL Medical  dated within one year of the tryout date AFTER July 1, 2025 and a completed/signed PSAL Parent Consent form. 

The PSAL Medical Form/Physical is approved for 1 calendar year but must be after July 1 2025. The PSAL Parental Consent Form must be completed and turned in for each sport/season.

• PSAL parent consent . 

You must turn in the original copies of the PSAL Medical and PSAL Parental Consent Forms to the coach or Athletic Director before the tryout date. You cannot participate in a PSAL sport without completed/approved PSAL Medical and Parental Consent forms. 

To ensure your PSAL medical form has been filled out properly, please go over this checklist:

• The medical form is filled out in pen and is 100% original. Copies, printouts, faxes, etc. cannot be accepted.

• The doctor has checked one of the boxes at the top of the page clearing you for participation

• The bottom of the form is signed, dated, and stamped by the doctor.

• A medical form is valid for one calendar year from the date the doctor puts on the bottom.
