All coaches are required to complete an Athletic Injury Report for any injury that occurs during a game. The report must be submitted to Anthony Carrano at acarrano@schools.nyc.gov with the subject line titled "Injury Report."
Home teams are responsible for ensuring that a first aid kit is on-site and CPR support is available during all games.

Transportation
Elementary: For a student to play a parent or guardian must bring and pick up the child from the game at the school they’re playing. Teachers or coaches are not permitted to bring the students to and or from the game. Please use a sign out sheet to know who was picked up. See a sample HERE.
Middle School: Coaches can transport students to the game and return them to their home school after the game. Parent pick up or student’s leaving on their own is based on the student’s blue card and parent consent. Free Subway Certificates so students and teams can ride for free can be applied for HERE. Sample transportation form from M.S. 936 can be found HERE. Please consider a travel ratio of 15:1
