When to submit a Consultation Request:
If your team/student has a more robust, ongoing need that would require student observation, team consultation, and possibly ongoing support, please complete the consultation request form
Note: parent permission is required.

Team Coaching
A referral must be placed for all students who do not have a current consultation agreement. The length of the consultation is determined by the team, and may last up to a school year. An exception may be made if the team determines that District Service Provider (DSP) support is needed for a grade-level transition across school years. If a current consultation agreement is open for a student, a new referral is not needed to request assistance from other DSPs. Students who have current support from DSPs written on their IEP do not automatically qualify for other IDT members' consultation. A referral must be placed for other DSPs when a current consultation agreement is not made for that school year. `
