Waivers, Fees & Forms

Please be sure to sign the electronic Friday Trip Prep/Catalina Trip waiver.

You will receive an email with the link to the waiver. Please read, sign and submit it online by Monday, 1/13/20. We cannot leave campus with your child without a waiver.

If your student requires any prescription medication, any over the counter medications or any remedies while on any SBMS trips, we need to know about it. A first aid teacher will hold and administer all medication. If any medication will be required for the March Expeditions, please return this completed form and medications required, clearly labeled, to the SBMS office by Friday, March 6, 2020.
