School Nurse

Welcome to the JBES Nurse's Office

Keep your child home when ill

If your child has had a high fever (100 degrees or higher) or has been vomiting, please keep home for 24 hours after the fever resolves without using fever-reducing medication.

Please notify the school nurse of any communicable disease such as strep throat, impetigo, chicken pox, or hepatitis.

Informing the nurse of your child's symptoms help us track any trends (see bottom of page) in our school community and caution others.

Free COVID-19 test kits are available in the Nurse's office for all students and staff.  Contact s_rodriguez@sau9.org to request.

If registering please give a copy to school of:

Medication

If your child requires prescription medications (including epi-pens, inhalers, and insulin) to be taken at school, the permission for medication at school form (link above) must be completed and signed by both the physician and the parent/guardian.

Medical Documentation

Immunizations

In accordance with New Hampshire State Law (RSA 141-C: 20-a) all students must present documentation of immunizations of certain diseases.  More information can be found here.

Information regarding exemptions can be found here.

Physical Examination

All students are to have a physical examination in accordance with ED311.03.  Proof of physical examination is a signed, dated summary of findings of a completed physical examination performed by a licensed health care provider.

A current physical (within 1 calendar year) should be on file.  Please send in a copy of the physicals when they are done.

Have any questions or concerns about your student?

Please let me know!
