All Families must fill out the sports permission form and turn it into the Athletic Director prior to the season starting. Also, all student-athletes must have a current physical on file in the Nurse's office. Physicals are good for one year, so please check the date. No Student will be allowed to participate in any practices or games without a permission slip and physical on file.
