Candidate Information Form 2023

Candidate Form

This is a required form for all new members of the National Honor Society. It allows for the selection committee to understand past leadership roles and to get a better idea of how the student is already a leader in their community.

2023 Community Recommendation Form

Community Recommendation Sheet

This is a required form for all new members of the National Honor Society. It requires that references be collected from TWO members of the community. These can be coaches, mentors, etc. We ask that this not be a parent or teachers. It allows the selection committee to develop a more well rounded picture of the student.
