Tryout Requirements

TRYOUT REQUIREMENTS: 

In order to tryout for any Gilbert H. Hood athletic team, or to participate in intramural sports, all athletes must:

1.  Have a completed Consent and Indemnity form (found below) turned in to the Athletic Director or Coach, on or before the first day of tryouts.

•  If this form is NOT completely filled out (including insurance information), your student-athlete will not be able to participate in tryouts.  

2.  Have a current physical on file with the school nurse

• THIS IS FOR PARTICIPATION IN  SPORTS ONLY

  • You only need ONE in your time at GHHMS.  Just make sure it is on file with the nurse.

3.  Completed Medical History Update form 

4. Come dressed appropriately and prepared with proper equipment for your sport/activity.
