The following clearances/forms must be submitted to the High School's Main Office before chaperoning an event. 

1. Volunteer Cover Sheet (with expiration dates)

2. Parent Acknowledgement of School Board Policy 916.1 (Renewed annually)

3. PDE 6004 Arrest Conviction Report (Renewed annually)

4. Act 114 - FBI Clearance OR Certification Form 10 Yr. Resident of PA (Renewed every 5 years)

5. Act 34 PA State Police Criminal Record Check (Renewed every 5 years)

6. Act 151 PA Child Abuse History (Renewed every 5 years)

7. TB Test Report (Renewed every 5 years)

All clearances should be submitted as one complete packet. 

When submitting for the first time, clearances cannot be more than one year old.
