Fingerprinting and Driver Form Information
To become a field trip volunteer (chaperone and/or driver), for Arcade Middle School, or any other school in the San Juan Unified School District, you must be fingerprinted per SJUSD Board Policy.


Fingerprinting
If you have never been fingerprinted as a volunteer for the San Juan Unified School District, you will need to come to the school office to receive a fingerprinting form. You cannot be printed without obtaining this form from the school. Fingerprinting is free, and can be done at Carmichael Box Shop, 4005 Manzanita Ave, Suite 6. Once you are printed, your clearance is good as long as you have a student in the San Juan district and commit no crimes.

NOTE: Fingerprint clearances are considered confidential information and thus are not transferred from school to school. So, as your child switches schools, you must contact each new school so they can request their own copy of the clearance.


Volunteer Driver Forms*
Any parent wishing to drive students on a field trip must fill out the Volunteer Vehicle Form. This form must be completed and received in the school office for Principal approval a minimum of 10 days before you plan to drive on a field trip. You must include a current copy of your insurance declaration page, (includes Policy number, vehicle covered, and coverage dates and limits) and a copy of your current drivers’ license.

*These forms must be renewed each year.
