Step 1: Open SDHS Enrollment Forms Document Link:

Step 2: Gather the following items for Enrollment Submission:

Please have the following information available:

  1. Child Name (First Name, Middle Name, Last Name)

  2. Child Date of Birth (DOB)

  3. Child Grade Level (example: 9th) for 2021-2022 School Year

  4. Has your child ever been enrolled in a SDUSD school?

  5. If you answered "yes" to #4 - what school(s) did they attend?

  6. Name/City/State of LAST school attended

  7. Parent/Guardian name

  8. Parent/Guardian phone number

  9. Home address

  10. Your relationship to Child. Example: Mother/Father or Guardian – Aunt/Uncle

Forms and items needed for enrollment (Please note: if any of these items are missing you will be rescheduled to an August enrollment date):


Form 1: 2021-22 SY PreK-12 Enrollment Form (Enrollment Form)

Form 2: Current Immunization Record Form (CAIR) or Immunization Record

Form 3: Health Information Exchange Form

Form 4: Universal Form

Form 5: Home Language Survey Form

Form 6: Caver 5 Expectations

Form 7: Request for Records (Eng/Span)

Form 8 Physical Education Exemption Form



One of the following utility bills in the parent/guardian’s name dated within the last 30 days:

a. Gas & Electric Bill

b. Phone Bill (Must be a landline, not cell phone)

c. Cable Bill

d. Water bill

e. Rental Agreement with a receipt for the last month payment

All Students must follow the SDUSD boundary residency guidelines or SDUSD CHOICE process.

Item 2: Copy of the Birth Certificate or Passport that verifies date of birth

Item 3: Copy of most recent report card or transcript

(Especially if your transcript shows any high school Math or World Language course credit earned in 7th and/or 8th grade).

Item 4: (if applicable): Most recent COPY OF IEP or 504

(Only if the student receives Special Education or 504 services)

Immunization Information:

New students to SDUSD will need to upload a copy of student immunization record.

Children entering grade 7 should have:

    • 4 DTaP (1 more required after the child's 7th birthday)

    • 4 Polio (3 doses meet requirement if at least one was given on or after the 2nd birthday)

    • 2 MMR (1st dose on or after 1st birthday)

    • 2 Varicella

Children entering grades 7-12 should have:

    • All of the above

    • 1 Tdap booster shot given after the 7th birthday.
