9. Additional documents required for submission are: Birth Certificate, Immunization Records, Transcript or Last Report Card, Proof of address (SDG&E, Lease/Rental Agreement, Mortgage Statement)

NOTE: If you are living with a relative, friend or renting a room - must bring all items listed below:

1. Homeowner's most recent utility bill (SDGE) and a copy of a valid CA ID

2. Your DMV License showing your name and address you provide

3. Current cell phone bill showing your name and address you provide OR any other bill with your name and address

4. Confirmation email or letter from the USPS of change of address

Enrollment Packet will be available for student/parents at school during scheduled enrollment appointments.
