  • You must also include a copy of one item of the following:

    • recent utility bill

    • rental agreement or rent payment receipts

    • property tax payment receipt

    • mortgage documents

    • military housing orders

• Copy of birth certificate, baptismal certificate, or passport

• Proof of all required immunizations (required immunizations listed below):

  • 4 Polio (3 doses if at least one was given on or after the 4th birthday)

  • 5 DTaP (4 doses if at least one was given on or after the 4th birthday)

  • 3 Hep B

  • 2 MMR (Both doses given on or after 1st birthday. Two doses of mumps, two doses of measles, and 1 dose of rubella vaccines are required if given separately.)

  • 2 Varicella
