Enrollment Process:
Step 1: Verify that SRHS is your neighborhood school by using the School Finder.
Step 2: Complete this Google Form to begin the enrollment process.
Step 3: Our Health Office will verify that your student's immunizations are current.
Step 4: We will email you the Enrollment Packet paperwork to complete and submit with the rest of the required documents (see list below).
If there are any questions regarding your current immunizations, the Health Office will contact you.
If your immunizations are current, you will receive an email with our Enrollment Packet.
When you return our Enrollment Packet via email, you must also include these documents as attached PDFs (unless you have already uploaded them via the Google Form):
Birth certificate or passport (must be scanned and attached as PDF, NOT a photo)
Current SDG&E bill for proof of residence (1st page only)
Most current Unofficial Transcript/Report Card (include Middle School if your transcript shows any high school Math or World Language course credit earned in 7th or 8th grade).
"Withdrawal/Drop Slip" if you are coming from another high school mid-year.
This slip lets us see how we can transfer your current classes/end grades to our classes.
*If applicable* Most recent IEP or 504 Plan

Immunizations (Must be verified by our Health Office before enrollment):
Children entering grade 7 should have:
4 DTaP (1 more required after the child's 7th birthday)
4 Polio (3 doses meet requirement if at least one was given on or after the 2nd birthday)
2 MMR (1st dose on or after 1st birthday)
1 Varicella
Children entering grades 7-12 should have:
All of the above
1 Tdap booster shot given after the 7th birthday.
