Required form for Epi-Pen and/or Benadryl

This form is required for any medication administration while at school. This includes short term antibiotics, eye drops, daily medications and any other over the counter medication needed 2+ times in the school year.

This form is for parents to share health history of their child, get help with insurance enrollment and give consent for their child to receive OTC medications.
