Diplomas are original documents issued to the students post graduation--the district does not maintain diploma copies; therefore, a duplicate cannot be provided.
Provide your name and date of birth exactly as it was during the time of enrollment.
Adult-age alumni must request their transcripts.
Background checks require a signed release and are not conducted over the phone.
An official photo identification is state or government-issued.
"Class of" or the last year you attended UCHS.
Accurate college/employers' information (name and complete mailing address, including zip code). All addresses provided by the requester will be cut & pasted as stated in the request.
All of the information requested must be provided in your submission---otherwise, the request is incomplete and will result in being delayed and/or may not be processed.
Student data, including attendance, grades, and transcripts, will be available through the PowerSchool Parent/Student Portal account. Otherwise, go through the University City High School webpage/Google Form linked below.
Unofficial transcripts can be accessed through the Parent or Student Portal.

Use Xello (for USA educational institutions).
Use Senior Google Classroom (for out-of-country educational institutions).

School sites do not have access to the scriborder processing, therefore it will be necessary to direct all questions to them directly at the contact information provided on their webpage.

Transcript requests must be submitted through the UCHS website's Transcript Request Form link, by completing the form and uploading an official photo identification (state or government-issued such as a driver's license, a passport, or military identification).

Educational Verification (Background Screening) requests must be faxed or emailed, providing the following information:
A signed authorization to release information
The student's name and date of birth exactly as it is on their school record
Contact information: An email address for the point of contact (agency representative)
Please note: Factors such as limited access to inactive student records in a database vs. actual archives, the volume of requests, peak-time/season, post school break/closure, etc., the five-business-day timeframe may differ as requests are processed in the order of receipt. Incomplete requests will not be processed.
